  Affidavit of_______________________ S/W/0_________________ward, ___________________________R/o______________________ I, the above named deponent, do hereby solemnly affirm and declare as under:-

  1.That I, the deponent am seeking admission in Ajit karam Singh International Pub. School, Sector_____Chd/Mhl/Gr.Mhl, herein also called AKSIPS----- of my ward namely ________________in the session _____________in class _______________in the Economically Weaker Section Category (EWS) of the School’s discretionary quota for EWS students.

  2.That I, the deponent undertake to furnish the following documents to the school authorities and the said documents may be treated as a part of the affidavit:-

  a)SDM Certificate (Income Proof of the Family)

  b)Proof of Residence.

  c)Original Birth Certificate of the student.

  d)Family income Certificate issued as well as signed by the employer and duly attested by a notary.

  e)Salary statement for last 3 years including the latest salary statement issued and attested by that employer.

  f)All bank Account statements for last 3 years of all family members.

  g)Copy of the latest Aadhar Card of all family members.

  h)Number of family members residing in the house with their names.

  i)Status of Residence:-Owned or Rented

  j)LPG Connection copy(Self attested)

  k)ITR copy for last 3 years ( if filed)

  l)Any other child taking same facility ( give details like school, name, class, admission year)

  m),Yellow Card(Only for Punjab)

  3.That I, the deponent am having health/life/or other policy in my name or in the name of my child/children/ spouse. I, the deponent am paying annual premium of Rs. ___________________towardsthe said policy /policies (Self Attested copies of policy/ receipt of premium paid to be attached)

  4.That if any statement made by me, the deponent in the affidavit or otherwise to the School Authorities while seeking admissio n or during the current year in AKSIPS, on a seat reserved for Economically Weaker Section turns out to be false, then the admission of the deponent’s ward will be considered as cancelled, and he/she be not allowed to continue even against General category seat. I, thedeponent will not raise any claim for seat either against reserved quota or general category. I, the deponent further undertake to pay the school authorities the fees payable by the student of general category from the date of admission till its cancellation and shall also be liable to be prosecuted in accordance with law.

  5.That I, the deponent further undertakes to intimate the school authorities regarding any enhancement in my income or my family income because of which I, the deponent cease to be covered by the definition of Economically Weaker Section of society as defined by the Law (RTE) or any other law or rule from time to time, and will pay the fee as applicable to a general category student from the date of their exclusion from EWS category. I will also be liable for legal action in case of any false information is given by me.
